AbstractWithin the scope of this work, a study was conducted on paramedical dermopigmentation, considered as an integrative tool of aesthetic rehabilitation for cicatricial deformities of the skin and defects of the nipple–areolar complex (NAC). The presentation is based on an interdisciplinary synthesis of histological patterns, biophysical principles, and clinical and practical observations, which makes it possible to substantiate a shift in emphasis from a decorative understanding of permanent makeup toward a reconstructive concept. The study demonstrates the specificity of pigment behavior in fibrotically altered tissues: the mechanisms of its retention are analyzed in detail with consideration of the macrophage renewal cycle, as well as the influence of optical light-scattering phenomena in anisotropically organized collagen structures that determine the perceived density and tone of pigmentation. A substantial part of the work is devoted to the author’s protocols of digital micropigmentation, the application of which ensured a 30% reduction in procedure duration while maintaining high reproducibility of the result and achieving a satisfaction index of 98% (n=500+), which received international recognition marked by the award of the Grand Prix. Current statistical data on the market for 2024–2025 are presented, demonstrating an exponential growth dynamic of the medical tattooing segment with an estimated CAGR of 9–11%. The publication is addressed to plastic surgeons, dermatologists, and specialists in aesthetic medicine and offers evidence-based algorithms for the management of patients with post-traumatic and postoperative defects.
